Program Agreement, Terms & Conditions:
I hereby acknowledge and agree:
1. The purpose of Nourished Life Healing counselling is to improve the overall health, vitality and well-being of the body, mind and energy through holistic healing practices. The holistic healer, Katie Sprague, does not diagnose diseases, disorders or conditions.
2. Katie Sprague, is not a Doctor or Medical Physician.
3. As part of the Holistic Counselling Services, I may be asked to provide information concerning my physical habits, medical history, moods, energy levels, likes and dislikes, lifestyle and diet. This information is collected to enable Katie to: (i) assess my knowledge of my overall wellness, (ii) educate me about the benefits of sound holistic wellness practices and (iii) recommend lifestyle changes to improve my general health, vitality and overall well-being. Katie Sprague will hold this information in confidence and will not release or disclose this information to any other person, without my prior consent, except as required by applicable law.
4. If Katie Sprague suspects the existence of disease, disorder or condition, I will be informed of this suspicion. However, I acknowledge this is not a diagnosis or conclusion about the state of my health and that I am directed to promptly consult a licensed Physician or Naturopath about any suspected problems.
5. Should I request Katie Sprague to recommend dietary changes and/or nutritional supplements to enhance my body’s natural ability to resist and/or overcome a known disease, disorder or condition, it is my responsibility to disclose the nature of the disease, disorder or condition and all other relevant details to Katie Sprague. If I have not previously consulted a licensed Physician or Naturopath about this disease, disorder or condition, I acknowledge that I am directed to promptly do so. I am not to alter or discontinue treatments prescribed by a licensed Naturopath, Physician or other licensed health professional without consulting the individual who prescribed the treatment.
6. In providing Nourished Life Healing Counselling Services to me, Katie Sprague is relying upon the truth, accuracy and completeness of all information I have provided to her. Any recommendations I follow for changes in diet or lifestyle are entirely my responsibility.
7. Katie Sprague is in no way liable for my health or safety.
8. In consideration of my participation in the Nourished Life Healing Counselling Services, I hereby accept all risk to my health, including injury or death that may result from such participation and I hereby release Katie Sprague, on my behalf and on behalf of my personal representatives, estate, heirs, next of kin, and assigns from any and all costs, claims, causes of action and damages arising from any and all illness or injury to my person, including my death, that may result from or occur as a result of my participation in the Nourished Life Healing Counselling Services, whether caused by negligence or otherwise.
9. I understand that payment for this service is $5000 for 6 months, and will be made upon signing up for the program. I understand that I have the option to pay for this service in the full amount specified at the time of booking, or to choose a payment plan option if discussed prior to purchase with Katie Sprague.
- I understand that when I pay for this service by credit card or debit card, I authorize and give permission to Nourished Life Healing, LLC to charge my credit or debit card for the amount owed for payment of the service booked. When I purchase the service, I understand that my information (i.e. credit card and contact info) may be collected by the third-party merchant Stripe, who may have privacy policies or security practices that are different than ours. Nourished Life Healing, LLC is not responsible for the merchant’s independent policies or practices.
- In the event that payment is not successfully made by the due date, I understand that I have a three (3) day grace period to make my payment for the service. If such payment is not received, I understand that my access to and participation in the service will not continue.

I understand that if I choose a payment plan for this service I hereby authorize and give permission to Nourished Life Healing, LLC to automatically charge my credit or debit card as payment for the service, for which I will receive an electronic receipt, at the time in which payment is due without any additional authorization from myself. I understand that I am responsible for completing all payments in the payment plan, regardless of if I am present for the sessions or not.

In the event that payment is not successfully made by the due date, I understand that I have a three (3) day grace period to make my payment. If such payment is not received, my access to and participation in the service will not continue. If I fail to make payment within the grace period or at any other time, I am still responsible for all payments for the full cost of the service.
10. 24 hours notice is required for cancelling appointments. For appointments cancelled within 24 hours of your appointment time, you will have one opportunity to re-schedule. After that, any further appointments that are cancelled within 24 hours of the appointment time will be considered "no-shows" and will not be re-scheduled.
11. Refund Policy: All purchases are final sale and no refunds are provided. If you have any questions or concerns, please email our team at [email protected].
12. Program Details: I understand that this is a 6 month program consisting of 6 60-minute sessions, 1 per month AND 2 additional sessions (90-120 minutes each) redeemable at the purchaser's preference throughout the 6 months. It is my responsibility to book my sessions and ensure that all of them are completed within the 6 month period. All sessions must be completed within 6 months of booking this program, unless otherwise specified and agreed upon due to vacation weeks, prior engagements, extenuating circumstances, or other. If there are leftover sessions at the end of 6 months, and an extenuating circumstance has not been discussed with Katie Sprague these sessions will be forfeited and there will be no reimbursement for them.
I understand that this package includes 2 days of instant messenger communication per week (M, Tu, Thu, Fri) via Voxer, between the hours of 11:30am-6:00pm EST. If I message Katie Sprague outside of those hours I understand that she will get back to me at her next available time. Katie Sprague has 24 hours to respond back to me during any of the allotted days and times.
13. Communication: I understand that it is my responsibility to communicate with Katie Sprague about my schedule. I may communicate with Katie via email ([email protected]) or via Voxer. If Katie Sprague has not heard from me for 2 weeks or greater during this 6 month program period, and there have been no extenuating circumstances discussed, she has the right to discontinue this package, without reimbursement.
14. I understand that any therapies I undertake at Nourished Life Healing LLC are undertaken of my own free will. I accept that the ultimate responsibility for my health care is my own and that Nourished Life Healing LLC is here to support me in this. I understand that my practitioner reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended. I hereby agree to assume full responsibility for any manner of loss, injury, claim or damage whatsoever, known or unknown, incurred as a result of same and I, my heirs, executors, administrators or assigns for any loss, injury, claim or damage sustained as a result of my attendance and/or participation. I have read the above release and waiver of liability, and fully understand its contents and voluntarily agree to the terms and conditions stated.
